About Living Hope Care

Our Story

 

At Living Hope Adult Daycare, we began with a simple mission: to support individuals in living safe, meaningful, and more independent lives. Today, through our Individual Residential Support Services (IRSS) and Supported Community Connections (SCC), we provide personalized, compassionate support that helps each person thrive at home and stay connected in their community. Our team is committed to delivering reliable, respectful, and professional services tailored to the unique needs and goals of every individual we serve.

Our Services

We offer a wide range of services including medication management, wound care, hospice care, and more. Our team of skilled professionals will work with you and your family to create a personalized care plan that meets your unique needs.

Host Home Providers

 

Our IRSS and SCC support staff are experienced, compassionate professionals who are carefully screened and trained to provide high-quality, person-centered services. Through Individual Residential Support Services (IRSS) and Supported Community Connections (SCC), our team is dedicated to helping individuals live safely in their homes and actively participate in their communities with dignity, respect, and independence.
