Enrollment at Living Hope Care

Enrollment begins with a referral from a physician or case manager. An initial phone consultation and/or in-person tour is then scheduled to review services and determine eligibility. If both parties agree to proceed, funding is arranged through Medicaid, long-term care insurance, or private pay options.
